Our office is open from 8 a.m. to 5 p.m., Monday through Friday. To minimize waiting, patients are seen by appointment, with the exception of emergency situations. To schedule or reschedule an appointment, please call us at 615.328.3380.

Missing or arriving late for appointments without advance notice can cause delays for other patients. As a courtesy we ask you to contact the practice 24 hours in-advance if you will not be able to keep your scheduled appointment date and time.  If you find that you must be late, please contact us as soon as possible so that we can determine if we need to reschedule your appointment. 

What to Bring

Photo ID, insurance card, and a method to pay your co-pay, which is due at time of service. Payments may be made by cash, check or credit card.

If you have had a X-ray or MRI for your problem, please bring a copy of the films (CD/DVD or printed out films) and the report if possible. If you use a brace (such as an ankle support) or orthotics, bring that as well.

If it is your first visit, has been more than a year since your last visit or if your information has changed, you can download our patient forms to fill out at home and bring to your appointment.
